What are the documents needed when a minor wants to obtain a passport?

- Letter from the doctor who delivered the child attesting to the birth.

What happens if I lost my Philippine passport?
If you lost your passport, you need to avail of a new one so you can legally travel in and outside the UAE. The first thing you need to do is to report the matter to the police. You will have a case certificate once the report is done and you have to bring it together with the other documents.
